Eligible participants can obtain benefits under your CWA Local 4319 Legal Services Plan by telephoning (419) 243-1105, identifying yourself as a Plan participant, and asking for an appointment. You may call any weekday between the hours of 8:30 a.m. and 5:00 p.m.
Lackey, Nusbaum, Harris, Reny & Torzewski L.P.A. is located at Two Maritime Plaza, Third Floor, Toledo, Ohio. It is especially important that you contact your attorney as soon as a legal problem arises. Your prompt action can make the difference between success and failure later on.
Any individual who is current dues paying member in good standing with CWA Local 4319, his or her spouse, and dependent children under the age of eighteen (18) who resides with the member are eligible for benefits as a participant, as provided in this summary Plan description. Retirees are also eligible to receive these benefits.
Your Legal Services Plan provides free legal consultation on any matter. A staff member is always available to serve you. The benefits provided free of charge are.
As a participant, you are entitled to receive unlimited free one-half hour consultation on any legal matter unless excluded below. This includes a review and discussion of your problem and your possible remedies. Examples of such covered matters are, consumer problems, personal injury, bankruptcy, domestic relations, and probate matters.
Each participant is entitled to the preparation of a simple will. In addition, a complete estate plan review will be conducted. This will insure that your life insurance beneficiaries are properly designated and any real estate is titled in the best possible form.
Estate planning will be provided to members and retirees at the rate of $75.00 per hour.
The firm will also prepare living wills for the member for an additional fee.
If you sell or buy a personal residence, the Plan will provide you with the necessary representation. This includes review and/or drafting of an Offer to Purchase, review of the title company's report and attendance at the closing itself. The expenses of title work or any other out of pocket expense will be paid by the seller and/or buyer.
Finally, participants of the Plan have available legal representation at refinancings for the fee of $75.00.
Since 1973, Ohio law has permitted married couples to hold property jointly with right of survivorship upon the death of their spouse. The Plan will review your deed and convert it if necessary. The only cost involved is the recording of the new deed.
Each participant is entitled to representation at actions commenced by the Bureau of Motor Vehicles for point violations, which includes the preparation of petitions for the fee of $75.00 plus filing fees.
In the event of a member or member's spouse is involved in an uncontested adoption, the fee shall be $175.00 plus filing costs to process the action from filing until completion.
The firm of Lackey, Nusbaum, Harris, Reny & Torzewski also provides representation in the areas of personal injury, workers' compensation, criminal defense, traffic matters, divorce, dissolution, custody, child support, probate, wrongful death, labor, DUI, small business and corporate and bankruptcy on an individual basis with the cost to the participant. The firm provides representation in these matters to participants in Northwestern Ohio. The hourly rate and costs vary depending on the nature of the action.

If any consultation on any legal matter you decide that you are in need of legal representation and it is a matter not covered under the benefits provided, your Legal Plan attorneys will be willing to represent you at a reasonable fee. All legal representation in matters not included in the schedule of benefits are provided solely at the expense of the participant and are not paid for or provided at the expense of CWA Local 4319.
